ALTERNATIVE TREATMENT OF CHRONIC CARDIAC FAILURE IN CHD PATIENTS WITH BRADICARDIA

   In spite of the progress achieved in treatment of patients with chronic heart failure (CHF) for last years, since now in practical activity of doctors during it's treatment of patients with bradicardia.

    Thus, such effective remedies like heart glycosides, are almost contra-indicated and diurethics usage quite often brings to refrac- tivity of the treatment. 

     Responding to clinical experience and literature database, we used naniprus (Natrium nitroprussidum) in patients with CHD and CHF (24 persons, 10 mails and 14 females in middle age 61,45 + 21,45) patients received naniprus in dose 30 mg/dose on background of concerning basis treatment. 

     Obtained results showed that naniprus has clinical and haemo- dinamical effect in 72% patients with CHF accompanying by bradicardia. Beginning from the eight day from preparation receivement positive dynamics of the following haemodinamics indexes: heart beat rate accelerated per 16,4 b/min in 29,6% cases, systolic pressure in pulmonary artery decreased for 39,4%. 

    So, our investigations are evidencing that Natrium nitropressidum usage in patients with CHF accompanied by bradicardia preconditions increase of diastolic engorge in left part of heart and causes decrease of venous return, blood engorge of pulmonary vessels, what accompanied by improvement of clinical condition of patients.

ANEMIA IN CHILDREN WITH RHOEVMATOID ARTHRITIS (RA)

    275 patients with RA under observation, in 120 of them (69,1%) anemia with different expression degree was revealed. With aim to study anemia development pathogenesis in patients with RA iron metabolism, immunologic, hemostatic, antioxidative systems state, haemolisis, investigation methods sinovia liquid and tissue were used. 

      It was established, that main link in anemia pathogenesis are deviations of immunity, especially it's humor link. Iron deficiency in patients with RA carries redisributable charac- ter. Antioxidant and oxidant systems balona dentations expressed in decrease of antioxidant activity correlated with iron metabolism dentation degree were discovered. 

     One of the leading links in anemia pathogenesis during RA. Infringement of erithropoesis humoral regulation expressed in erithropoesis inhibitor activation increase blood's erithropoetical function deactivation and middle molecules amount increase is one of the leading links in anemia pathogenesis during RA. 

    Our investigations are giving us reason to consider that anemia carries complex multi-factoral character in patients with RA heading link in anemia development in patients with RA belongs to immunity changing.

CAPOTEN AGAINST CORVATON IN TREATMENT OF THE CHD AND HEART FAILURE PATIENTS WITH PACEMAKERS

    With the purpose of definition of influence of long application of Captoprilum (capoten of firm Bristol-Myers Squibb, USA) on a survival rate and the current of a circulatory unefficiency at the patients CHD with a heart failure 1-2 FC (NIHA) taking place in various regiments of electrocardiostimulation (ECS) was carried out a research on 28 patients (1-st group) on the average age of 58,4 + 3,2 years. 

    All patients received for a long time, till 5 years, capoten + diuretics (if necessary with addition of digoxine). 2 groups were made of 30 patients of CHD with a heart failure 1-3 FC (NIHA) on the average age of 56,2 + 2,7 years are received for a long time corvaton + diuretics (if necessary with addition of digoxine) also with in 5 years and also taking place in various regiments of ECS. 

     Methods of the control -veloergomentry, spiroergo- metry, echocardiography, right-hand catheterization of heart. The analysis of survival rate was made by a method Kaplan-Meier. It is revealed, that at the patients of CHD with ECS it is necessary to consider a combination of substantial growth of the sizes both volumes of a left ventricle and left auricle by raised level of diastolic pressure in a pulmonary artery, excessive augmentation of an index of a stroke output left and, in particular, of right ventricles of heart, low values of pulmonary capillary wedge pressure and combination dp/dt max in early periods after an implantation of ECS, observed already in rest, prognostically adverse by way of a survival rate in the remote terms irrespective of a maintenance therapy. 

     It is also revealed, that in the 2-nd group 78,2% by the patient was necessary to add in treatment the reception of digoxine whereas in the 1-st group -42,4%. The survival rate of the patients of 1-st group was authentically above in comparison with the patients of the 2-nd group.

COMBINED ELECTRONARCOSIS INFLUENCE ON SOME VALUES OF CENTRAL PERIPHERIC HEMODYNAMICS IN PATIENTS IN CONDITIONS OF SEVERE HEMORRHAGE

     In 178 patients with severe hemorrhage using native apparetus for electroanesthesia "Elektronarkon-1" combined electronarcosis (CEN) was carried out.

    On the background of CEN values of central and peripheric hemmodinammics were studied and also the volume of circulating blood (VCB) and its components were studied.

    A conclusion was made, that CEN was a kind of anesthesia providing adequecy of anesthesia. Toxic and depressing absence of influence on central hemodynamics, compensator-adaptation presentation of reaktion to hemorrhage made it a safe kind of anesthesia in urgent situations on the background of severe and masive hemorrhage.

 

 

 

 

IDENTIFICATION OF HUMERAL AN CELLS INDEXES IN THE PATIENTS WITH CARDIOPULMONARY INSUFFICIENCY ACCORDING TO STAGES OF THE ILLNESS

     Our observations covered 17 patients at the age of nearly 51,0 + 16,4 (8 women and 9 men with the cardiopulmonary insufficiency and also chronic obstructive bronchitis (6), bronchial asthma (5), lungs emphysema's (6). 

   In order to evaluate the immunological reactivity of the patients, the quantity and functional activity of cells (E) and (B) humoral parts of immunological systems have been studied. As a result of the investigation of the immunological status, it was discovered that, in the beginning stage of cardiopulmonary insuffi- ciency general T lymphocytes have been decreased for 43% (P<0,05), T-helper/T-suppressor indexes have been increased for 11%, VgM has been decreased for 19% (P< 0,01) and immunological complexes circu- lating in the blood have been increased for 21% (P<0,05). In the second and third stages of cardiopulmonary insufficiency the abovementioned indexes have gone worse, JgA and immunologcal complexes have decreased. 

   The analyses of the results showed that on a background of attenuation of T-population in the chronicle cardiopulmonary insufficiency, the following are observed: inhibition of T-suppressor activity, proliferation of B-lymphosits, increase of antibodies of JgA. As a result of our investigation it was decided, that beginning from the first stage of cardiopulmonary insufficiency the body is loaded with antigents and tolerance is upset.
