STRUCTURE BASIC OF HIGH HYDROSTATIC PRESSURE SUPPORT IN ENDONEWRAL SPACE OF PERIPHERAL NERVES

      According to modern demands before morphologists are staing objectives of complex study of structure elements of different organs and their liquids. On this issul, Especial peace are talking bunches of nerve fibers (absence of lymphatic vessels, presence of hematonewcal and perinewral barriers) inside of peripheral nerves. 

     In this paper we tried to generalize morphological data gained by us about hystotopograpgy and ultratructural organization of endonewral microvessels and their correlation with perinewral membrane of peripheral nerves bunches. In the present work light optical and electronic-microscopy methods were used

    It was noticed that one of the original especialities of microvessel net of endonewral area is opresence of capillaries with wide caliber (7,6- 0,5 mcm) and almost continuous layer of perycytes, that trans for to member of thin-walled postcapillar venules.

     Talking into attention that hydraulic conductivity of venule is more than microvessels wall capillary wall it can be considered that flow of blood under high pressure as a result of not very high resistance in endonewral capillaries into postcapillar venules lets to keep hydration of endonewral area on the necessary level.

     Besides that, follow of thin-walls endonewral venules in rough "rigid" perinewral membrane may play important role in generation of postcapillar resistance size, that has an influence on size of liquid filtration, what is necessary for keeping high hydrostatic pressure in interstitial space of endonevrium.

SIGNIFICANCY OF COMPUTER BASED TOMOGRAPHY IN DIAGNOSTICS OF OTOGENIC BRAIN ABSCESS

     Aim of the present work is to study comparison results of computer based tomography data about brain abscess localization with topical brain damage symptoms and identification of computer tomography possibilities in determination of abscess via punction through ear trepanation wound,

    To achieve the goals we studied case histories of 94 patients with otogenic cerebra and cerebella abscess in age form 1- till 59 .

    In 92 patients brain abscess developed as a result of chronical supurative epitimpanitis, in 1- due to acutization of chronic mezotimpanitis and in 1 patient due to activation of acute supurative otitis media . In 33 patients brain abscess localization place was left hemisphere, in 28- right hemisphere , in 33 - cerebellum.

     Comparative analysis of focal symptoms showed that amnesic and sensor aphasia are typical for left temporal, temporo-cranial lobe, while vestibule disorders, apractognostic symptom, memory and mental disorders are typical for right hemisphere abscess. Aphasia noticed rarely. Motion disorders Apraksia, amnesia, agnosia, asomatognosia, were noticed in all patients.

    Common computer based tomography sign of abscess formulation is round and ellipse-shaped low-density formations presence in center according to purulent content and ring-shaped high-density shadows appearance in peripheral part concerning to abscess capsule. Computer based tomography helped to reveal changes in brain tissue after pus elimination . Comparision results of clinical- rentgenological data gave us the reason to devide 2 kinds of abscess localization in cerebellum: frontointernal and dorsolateral ones. Focal symptoms of cerebellar injury were revealed during abscess localization in it's frontointernal part. On dorsolateral cerebellar abscess locatoion topical symptoms were absent.

     Otogenic abscess diagnosis was finally established by brain punction and proved by getting of pus during operative invasion. But punction of necessary brain area via ear trapanative wound process conducted traditionally was oftenly difficult, even after focal symptoms and computer based tomography method's data obtain about the process localization.

      Case history analysis showed that the method of brain abscess investigation and definition of brain abscess localization through punction conducted on base of computer tomography method data , proposed by prof.A.T. Talishinsky, makes possible exact definition of brain abscess , helps to prevent ventricle and brain vessels injury and to sanate abscess more effectively.

     Use of this method made possible find the process from 1 st trial in more than 60 patients.
